This Learning and Sharing Forum is part of SCIC Technical and Process Safety Committee’s initiative to provide a platform of sharing and information exchange on good practices and implementation experiences among the Chemical Industry.

Simply attributing incidents/accidents to human error is not adequate; human factors aspects should be investigated thoroughly such that lessons are learned to prevent recurrence. A key point of a human factors analysis is to understand why actions and assessments that are now (after incident) controversial, made sense to people at that time (before incident). There are tools available to guide this investigation process systematically to identify the underlying reasons for the human behaviours and errors.

By participating in this forum, attendees can gain and share valuable insights and practical knowledge, enabling them to implement and enforce Incident Human Factor Analysis effectively shifting the paradigm from person focused approach (human error is a cause of failure) to system focused approach (human error is a symptom of failure) in their respective organization, thereby ensuring a safer workplace for all.
