An initiative of the SCIC Technical and Process Safety Committee -  Learning and Sharing Forum – Integrating Human Factors in Incident Investigation

 

The Learning and Sharing Forum held on 30 May 2025, brought together process safety professionals in a highly engaging session hosted at e2i @ Devan Nair Institute. Organised by the SCIC Technical and Process Safety Committee, the forum was part of the capacity building effort to foster meaningful knowledge exchange and promote best practices across the Chemical Industry.

With around 40 participants, the session offered an interactive platform for case study discussions and deeper exploration into the role of human factors in incident investigations.

Participants were introduced to tools and methodologies that support systematic human factors analysis, helping to uncover underlying systemic issues rather than placing the blame on individuals. This approach shifts the focus from a person-focused model where human error is seen as the cause of failure to a system- focused model where human error is viewed as a symptom of deeper organisational or process-related issues.

Overall, the attendees gained practical insights and shared experiences reinforcing the value of adopting human factor lens in safety management. By applying these principles, organisations can better prevent recurrence of incidents. Stay tuned for more forums throughout 2025.
