Workshop on “Conducting an Effective Incident Investigation”
SCIC’s Process and Engineering Committee is organising a Workshop on “Conducting an Effective Incident Investigation”.
This half-day workshop provides participants with information needed to properly investigate incidents involving chemical processing facilities. Participants will learn about the objectives of investigation, conducting witness interviews, importance of properly collecting, documenting and retaining evidence, and determining the root cause(s) of incidents. Participants will also collaborate in small investigative teams to analyse incident case studies.
This workshop is valuable to company personnel from operations, engineering and safety. This workshop incorporates the use of the “Five Whys” method as it is the easiest to pick up and implement, especially for lower level investigations. A more thorough and structured method, Fault Tree Analysis, will also be covered.
• have an understanding of the relationship between major incidents, minor incidents and near misses
• be able to identify the direct and indirect causes of incidents
• be able to record the findings of an investigation
• be able to effectively undertake an investigation
